Proof of negative pre-departure COVID-19 test results
Travelers are required to take another 2 COVID-19 PCR tests in Honolulu on day 5 and 48 hours before departure to American Samoa.
The test results are automatically updated to the TALO PASS registration.

Mandatory travel form
Travelers are required to submit the TALOFA PASS for approval of the American Samoa Government. The final health questionnaire has to be completed 24 hours before departure which will generate a QR code.
Nationals and residents entering American Samoa must apply for repatriation with the Department of Health at least 21 days before departure to American Samoa.
Mandatory health forms for residents
Nationals and residents entering American Samoa must provide a travel declaration form and a health clearance by a licensed physician.
Mandatory proof of vaccination for American Samoa
Fully vaccinated travelers are required to present a valid vaccination certificate showing that 14 or more days have passed since the final administration of the vaccine.
This requirement does not apply to:
- Travelers under the age of 5.

Travelers are required to quarantine for up to 14 days on arrival.
