To complete the form, travelers must upload a passport copy, a selfie photo, contact information, flight and accommodation details, proof of negative COVID-19 PCR test results taken a maximum of 72 hours before departure, proof of COVID-19 vaccination (if applicable), and proof of health insurance.

For travelers under the age of 18, the legal guardian, parent, or accompanying adult is responsible for completing their application.
